I understand that I am responsible for returning all Omaha Public Library materials borrowed on my Library card on time and in good condition to avoid fines, damaged or lost fees.
I agree to pay the applicable fines/fees if materials are damaged, lost, or returned late and I understand that unpaid fines and fees may be turned over to a collection agency.
I understand that it is my responsibility to contact Omaha Public Library immediately if my Library card is lost or stolen to prevent unauthorized use.
I understand that it is my responsibility to give immediate notice of changes in address, telephone numbers or email.
I understand that the Library will attempt to contact me regarding items being held for me or items that are overdue, but that such notification is a courtesy and is not guaranteed.
